Mission

a Grace PLACE enhances the lives of adults and their caregivers by providing individually-focused daytime services to support community based living.

Services Offered

At a Grace PLACE, we provide daytime heath services and support for adults with disabilities and chronic and/or age-related conditions, while providing respite and support for their caregivers. Our center is open Monday through Friday, from 7 a.m. to 6 p.m. (closed on the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day & Friday following, Christmas Eve, Christmas Day). To support the many needs of our diverse population, we provide four program models for our members (descriptions and links to individual program pages found in the box to the right).

Scope of program services: Integral to all of our program models is Person-Centered planning for the development of each individual’s care plan, which provides a higher than average staff-to-client ratio and greater individual achievements.

Vital to the success of our Person-Centered planning are wrap-around services that include:

Nursing and Health Related Services for routine monitoring, medication management and health maintenance is incorporated to prevent physical deterioration and enable adults to enjoy home and community life.

A Nutrition and Wellness Program, for meals and snacks according to special diets which often provide the only nutritious meal(s) received by the client during the day. During FY2007 we provided 25,000 meals and 47,000 snacks.

Therapeutic Recreation Program, which focuses on increasing socialization, friendships, and a quality of life that builds hope. Older adults are socially and emotionally supported through Recreation Therapy, acceptance and a sense of belonging, and opportunities to interact and build relationships with others.

Caregivers, their families and the public at large are supported through our Care Giver Education Program. We work toward independence of meeting basic needs by teaching daily living skills and, as necessary, providing direct assistance in meeting personal care needs to ensure that the caregiver has the skills necessary to assist at home.

Our best practices Creative Arts Program has shown success in client engagement, self-esteem and self-expression. The arts are a powerful therapeutic tool and non-medical way to help improve mood and behaviors for those in the grip of dementia, Alzheimer’s or other cognitive impairments. Both music and art engage the parts of the brain that remain intact long after the onset of dementia.

a Grace PLACE began in the 1960’s as one man’s search for community and has grown into a source of hope for hundreds of families. Founder Louis Michaux’s own experience with cerebral palsy gave him the understanding that people with disabilities were searching for day resources offering the opportunity for socialization, cognitive and intellectual stimulation, basic health maintenance, and assistance with daily living activities. Today at a Grace PLACE, we celebrate a rich tradition of providing daytime health services and support for adults with disabilities and chronic age-related health conditions, while providing respite for their caregiving families.
